refactor: 技能統一收進 jsc-skills，加入 herdr，版本統一為 0.0.1
- 新增 jsc-skills plugin，含 herdr、herdr-issue、ingest，以 /jsc-skills:<名稱> 呼叫
- ingest 自 companion 移入，移除獨立的 herdr-issue plugin
- companion 版本由 0.0.3 改為 0.0.1

fix: companion 改用 $.fs，修正 hooks 模組載入失敗
node-fs.ts 引用 node:fs/promises，hooks 沙箱不允許，導致 /companion 指令未註冊。
改為以引擎的 $.fs 包成 Fs adapter；rename 改為選用，無 rename 時直接覆寫。
版本號調整為 0.0.1。

refactor: 拆分 cockpit 為可個別安裝的 statusbar 與 companion
- statusbar：輸入框上方狀態列，指令 /statusbar
- companion：小說角色陪伴，指令 /companion，自行渲染角色行
- companion 資料目錄改為 ~/.companion
- 整併 M4 檔案至 companion；M2 實作移至 hooks/companion/persona
- README 移除本機資料夾安裝，補上更新與移除指令
