以 mod 事件邊界測試補上輸出改寫的邊界行為：長回覆 Sonnet／一般 Haiku、逾時或額度不足顯示原文、/config 關閉原文 log 時不寫入、前綴只加在整則回覆開頭一次。實測皆符合規格，無需修改實作；claude plugin test 55 項全數通過。

test: 補測輸出改寫邊界行為（模型選擇、逾時與額度不足、原文 log 開關、前綴只加一次） 535c5b1437 
實測行為皆符合規格，無需修正實作。
